IMPORTANT INFORMATION
DOCTORS
Duff Street Medical and/or its representatives provide Practice Management Services to your treating doctor as instructed. The treating doctor remains solely responsible for their conduct towards you, independent of the Practice Management Services provided.
HOME VISITS & AFTER-HOURS CARE
In case of emergency clinic closure, call 000 or go to the nearest hospital with an emergency department. Home visits may be available after clinic hours if deemed appropriate by the doctor. Please speak to the Receptionist for details. For after-hours home visits, contact DR DR (locum service) at 132660.
PRACTICE COMMUNICATION POLICY
Our Receptionists are not medically trained to give advice over the phone. You may ask to speak to the Doctor in the case of an emergency. Where possible, your call will be put through to the Doctor or, if unavailable, to the Practice Nurse.
PROCESS FOR FOLLOW UP OF RESULTS
No results are given over the phone. Patients are required to make an appointment to discuss results with a Doctor.
MANAGEMENT OF PATIENT HEALTH INFORMATION
We comply with the Health Records Act (Vic. 2001) and the Privacy Act (Commonwealth 1988) to safeguard your medical records. Your information is kept confidential and accessible only to authorized personnel involved in patient care. To transfer your records, complete an information release form and provide a copy of your photo ID for privacy and confidentiality.
NON MEDICARE CARD HOLDERS
Additional charges apply for non-Medicare card holders, including during and outside regular hours.
