Our Diabetic Clinic at Cranbourne is committed to providing comprehensive assessment, diabetes screenings, and treatment to our patients. We understand that diabetes is a chronic condition where insulin, a hormone that converts glucose (sugar) from food into energy, is no longer produced or is not produced in sufficient amounts by the body. An estimated 280 Australians are diagnosed with diabetes every day, which makes diabetes the fastest-growing chronic disease and is the sixth leading cause of death in Australia.
Duff Street Medical Clinic is committed to the early prevention and detection of diabetes (up to 60% of cases of Type 2 diabetes can be prevented), as well as assisting our diabetic patients to stay informed and manage their condition.
We provide a range of diabetes care services and plans at our Cranbourne clinic:
Diabetes General Practitioner Management Plan (GPMP) – this care plan allows you to have up to 5 bulk-billed visits to a range of allied health professionals, including diabetic educator, podiatrist, dietician, and physiotherapist. This care plan is reviewed annually.
Diabetic Cycle of Care – this is an ongoing treatment program run in partnership with your regular GP and one of our experienced practice nurses. Throughout this course of treatment, your practice nurse will:
Assess your diabetes control by measuring your HbA1c levels
Ensure that a comprehensive eye examination is carried out
Measure weight and height and calculate your BMI
Ensure that a comprehensive foot examination is carried out
Measure total cholesterol, triglycerides and HDL cholesterol
Test for microalbuminuria (albumin in the urine, which can be a risk factor for cardiovascular disease and kidney disease)
Educate you regarding diabetes management
Review your eating and exercise
Review your medication
